Lymphoedema vs Lipoedema: Understanding the Difference

All information in this blog is supported by clinical guidelines and peer-reviewed research. There is currently no dedicated NICE guideline for either condition, so current evidence is used throughout. Reference numbers appear throughout, with the full list at the bottom of the page.



Two Conditions, Frequently Confused

Lymphoedema and lipoedema are both chronic, debilitating conditions affecting the limbs, and they can genuinely look similar at first glance, both cause swelling, both predominantly affect women, and both significantly impact quality of life. ¹ But they are fundamentally different conditions, with different causes, different patterns, and different treatment approaches. Genuinely importantly, lipoedema is frequently misdiagnosed as obesity or lymphoedema, leading to ineffective or even inappropriate treatment. ² ³ Getting an accurate diagnosis is the essential first step.



What Is Lymphoedema?

Lymphoedema is fluid-based swelling, caused by a damaged or overloaded lymphatic system, the network of vessels responsible for draining excess fluid from body tissues. ⁴ It can be primary, meaning inherited, but far more often it is secondary, developing as a result of damage to the lymphatic system, most notably following cancer treatment, such as lymph node removal or radiotherapy. ⁵



What Is Lipoedema?

Lipoedema is a hereditary fat-storage disorder, an abnormal, disproportionate accumulation of subcutaneous fat, primarily affecting the lower body, hips, thighs, buttocks, and sometimes the arms. ¹ ⁶ It's thought to be hormonally driven, often first appearing or worsening during puberty, pregnancy, or menopause, and current research estimates it affects 11 to 15% of adult women. ²


A genuinely key distinguishing feature is that, unlike fat related to general weight gain, lipoedema fat is resistant to weight loss or limb elevation, neither dieting nor elevating the affected limb meaningfully reduces it. ⁵

Why Does This Distinction Matter So Much?

Beyond simple curiosity, an accurate diagnosis genuinely changes the treatment approach. Lymphoedema and lipoedema are distinct conditions requiring different diagnostic approaches and treatment strategies, with complex decongestive therapy being the established cornerstone for lymphoedema management, while lipoedema often requires more specialised, fat-targeting interventions. ⁸


It's worth being honest that complex decongestive therapy, whilst proposed by guidelines as a first-line conservative treatment for both conditions, shows genuinely limited benefit in pure lipoedema where there is no actual fluid excess or lymphatic dysfunction present. ⁶ This is exactly why getting the right diagnosis matters, treating lipoedema as though it were purely a fluid problem will likely be disappointing.



What to Look Out For

Please seek medical assessment if you experience:

  • Sudden, rapidly worsening swelling in one limb (which may indicate a blood clot, requiring urgent assessment)

  • Redness, warmth, or fever alongside swelling (possible infection, known as cellulitis, which can be a genuine emergency in lymphoedema)

  • Skin breakdown, weeping, or open wounds

  • New, unexplained swelling without a clear cause



How Is It Diagnosed?

Diagnosis relies primarily on a thorough clinical history and examination, looking carefully at the pattern, symmetry, and texture of the swelling, alongside whether the hands and feet are involved. Imaging may be requested where the diagnosis remains unclear, and current guidance suggests insisting on further imaging if your diagnosis isn't clear from clinical assessment alone. ⁹ It's genuinely worth knowing that both conditions can occur together, and patients with both obesity and lipoedema present a distinct clinical challenge requiring careful, individualised assessment. ²

Lymphatic drainage massage: a core, well-established component of complex decongestive therapy for lymphoedema, helping to manually encourage fluid movement through the remaining functional lymphatic pathways.


Compression therapy guidance: well-fitted compression garments genuinely benefit both conditions, though it's worth being realistic that compression won't shrink lipoedemic fat in the way it can help manage fluid in lymphoedema. ¹ Pneumatic compression can also support complex decongestive therapy by improving venous return. ⁶


Exercise and movement guidance: forming part of the broader complex decongestive therapy approach for both conditions, alongside skincare and compression. ⁶


Referral and specialist coordination: given the complexity of these conditions, particularly lipoedema, which is so frequently misdiagnosed, we will always ensure you're appropriately referred to specialist lymphoedema or lipoedema services where needed, working alongside, rather than instead of, that specialist care.



Living With Lymphoedema or Lipoedema: Practical Advice

  • Skin care matters genuinely for both conditions: neglecting skin care increases infection risk in lymphoedema, and worsens skin changes in both conditions, so a consistent, gentle skincare routine is a meaningful part of self-management. ⁹

  • Consistency with compression is essential: continuous therapy with strict compliance is important, premature discontinuation of compression therapy is a recognised, common pitfall that undermines progress. ⁹

  • Don't assume it's "just weight": if you have disproportionate, symmetrical lower body swelling that hasn't responded to weight loss efforts, and particularly if your hands and feet have remained unaffected, it's worth seeking assessment for lipoedema specifically, rather than continuing with weight-loss-focused approaches alone.

  • Look out for associated conditions: both lymphoedema and lipoedema can have associated conditions that worsen the overall picture if left unaddressed, so a thorough, ongoing assessment matters. ⁹



What to Expect at Your First Appointment

We will carry out a careful clinical examination looking at the distribution, symmetry, and texture of the affected areas. We will explain clearly what we find, and where appropriate, support referral to specialist lymphoedema or lipoedema services, while providing complementary, supportive treatment such as lymphatic drainage massage and compression guidance.



Frequently Asked Questions

Could I have both conditions at once? Yes, genuinely. Lipoedema and obesity-related lymphoedema often appear together, and both are typically managed by the same healthcare professionals working collaboratively.


Will losing weight fix my swelling? It depends which condition you have. Lymphoedema-related swelling can sometimes improve with overall weight management, but lipoedema fat is specifically resistant to weight loss, which is exactly why an accurate diagnosis matters so much.


Is this just "stubborn fat"? If it's lipoedema, no, it's a recognised, hereditary medical condition, not simply excess fat from lifestyle factors, and it deserves to be taken seriously and treated appropriately rather than dismissed.


Will compression garments help? Yes, for both conditions, though the goal differs slightly, compression helps manage fluid in lymphoedema and supports overall tissue health in lipoedema, but it won't shrink lipoedemic fat itself.
